抄録 | 薬剤師の在宅患者に対するアセスメント向上を目的として,シナリオとアセスメントシートを組み合わせたシナリオトレーニングおよびフィジカルフィギュアを用いた教育を,薬局に勤務する薬剤師6 名に実施し,教育効果を調べた.講習前では,測定項目として多くの薬剤師が脈拍,血圧,呼吸数,体温および浮腫の5 項目を挙げていたが,心音および肺音を測定する薬剤師はいなかった.バイタルサインの測定意義および測定の技術講習実施後のシナリオトレーニングでは全員が心音および肺音を聴取した.最も可能性の高い疾患として,講習後では全員がうっ血性心不全と回答した.講習後のアンケート調査では,講習前と比較して,心音聴取の意義および異常音の区別の項目で取得度が有意に増加した(P <0.05).これらの結果より,本教育は在宅医療を始める薬剤師にとって有用な教育であると思われた.We conducted an educational program (EP) to increase the assessment skills of pharmacists visiting home care patients. The EP has two components. The first component is training that combines a congestive heart failure case scenario and patient assessment sheets. The second component is a lecture about the significance of measuring vital signs and methods for measuring the signs. Both components use the Physical Figures. We conducted the EP with six pharmacy based pharmacists, and examined the educational outcomes. The evaluation before the lecture showed that most of the pharmacists indicated five items to measure: pulse, blood
pressure, breathing rate, body temperature, and edema. None of the pharmacists listened to heart and lung sounds of the Physical Figures. After the lecture in the second component, all six pharmacists listened to the sounds of the heart and lung. Additionally, all answered that congestive heart failure should be the most highly suspected disease in the scenario. The questionnaire survey after the lecture showed that the level of knowledge of the pharmacists increased significantly regarding the importance of listening to heart sounds, and noting the distinction between normal and abnormal heart sounds, as compared to before the lecture. These results showed the effectiveness of the EP for pharmacists who will begin visiting home care patients. |
